All registration information and comments collected by this site are confidential and used solely for the purposes of this special project with the Centers for Medicare & Medicaid Services.  While we may develop aggregate level reports based on submissions to this site, we will never disclose your individual-level data without your consent.  If you have any questions about confidentiality, please contact us.
